Claim forms covering any illness or injury will not be accepted by the Employer and the Employee will not be eligible for Short Term Disability benefits for the absence in question, unless the form is submitted within fourteen (14) calendar days from the date the Employee's obligation to provide documented verification for their absence first arose under the Regulations to Appendix "C".
